Job summary

The Case Management Coordinator is responsible to work collaboratively with the Director of Youth Care Services to ensure the care management system is followed for every youth residing in a HomeBridge program. The Case Management Coordinator will work directly with both the HomeBridge program Supervisor, as well as liaise with the youth’s Social Worker and the anyone involved in the young person’s network of services and supports. The Case Management Coordinator will be involved in care management in varying capacities within different HomeBridge programs and may be expected to report to any of the HomeBridge programs.

Required Qualifications and Skills

● Master’s degree in Social Work plus 3 years’ experience in child protection contexts preferred. Registered with the Nova Scotia College of Social Workers (NSCSW). ● Master’s degree in Child and Youth Care plus 3 years’ experience working with children or youth in a child caring setting. ● Bachelor of Social Worker plus 5 years’ experience working in child protection contexts preferred. Registered with the Nova Scotia College of Social Workers (NSCSW). ● Demonstrated knowledge of the Department of Opportunities and Social Development care planning and care management systems. ● Access to a vehicle and have a valid driver's license.

Required Experience

● Understanding of a Child and Youth Care Approach. ● Knowledge of the Children and Family Services Act. ● Knowledge of Child and Family Wellbeing Policies and Procedures. ● Profound understanding of the needs of marginalized and discriminated communities and families.

Responsibility

● Meet with the Director of Youth Care Services minimally once a week to review and discuss the needs of various youth within HomeBridge Youth Society. ● Under the supervision of the Director of Youth Care Services, be responsible for the complex care management of youth where required. ● Assist in the care management of all youth within HomeBridge Youth Society. ● Review reports of all HomeBridge programs and read youth logs as necessary. ● Research and develop a model which allows concise and easily accessible care management information in relation to all youth within HomeBridge Youth Society in consultation with the Director of Youth Care Services. ● Liaison and build relationships with all service providers involved with HomeBridge youth as well as the Department of Education, Justice, Health, Community Services and other community organizations. ● Attend youth care team meetings as requested by the program Supervisor. ● Ensure appropriate information is documented and accessible to youth care teams in relation to care management. ● Involvement in the transition planning for youth as they both enter and leave HomeBridge Youth Society as required. ● Articulate and apply various social work theories within the context of youth care practice. ● Attend and support Youth Care Workers in care management meetings and other relevant areas in relation care management. ● Support Youth Care Workers with navigating and understanding the different systems involved with the youth including Justice, Health, Education and other Community Services. ● Ensure Social Work theories are considered within the development and implementation of care plans and interventions. ● other duties as assigned

Organization Overview

The work environment demands strong concentration and the ability to focus while maintaining sensitivity and appreciation of the delicate balance required in working with the clientele. Work hours are based upon client needs, typically Monday to Friday during business hours. Occasional evening or weekend work may arise. Must be able to work independently, and under stressful conditions. The working environment often requires sensitivity to cultural, societal differences and appropriate protocols. Working conditions can include physical, emotional and mental demands.
